Membership Volunteer Self Assessment form

We are asking all members to complete this short self assessment form in order for the committee to assess all members volunteer hours for the year so far and also the volunteer policy

The form below will ask you to estimate hours of volunteer across a number of activities.

NB Disney on Ice is NOT included in this survey as volunteering in relation to Disney on Ice (and its associated fundraising and events) directly relates to to individuals participation in the event.

It is important that each family complete the form. If the form is not completed the hours would be assessed as zero. It is appreciated that all families take the time to complete this information.
